Hi, good afternoon. I'm Debbie Margulies. I'm an endocrinologist from New Jersey. Welcome to red herrings and thyroid care. We have three great speakers today. It's going to be fun and informative session. I'm going to introduce all the speakers now. We're going to have questions right after the first talk for Dr. Spencer. She's going to have to take off right after and then we're going to have Drs. Barbicino and Dr. Cotwall speak before we have a question and answer session for them. So Dr. Carol Spencer is professor of medicine at the University of Southern California where she's director of the USC endocrine services lab. Her research career has focused on the clinical and lab aspects of thyroid disease and treatment. Her list of awards accomplishments and publications is long and distinguished and too long to list here today. We're lucky to have her here with us and she's going to be teaching us about pitfalls in thyroid testing. Welcome, Dr. Spencer. Thank you very much. Can everybody hear me? Okay. So I'm talking about pitfalls in thyroid testing. I have no disclosures and I'm going to focus on TSH, the free thyroid hormones, the thyroid antibodies, TPO antibody and thyroglobulin antibody and thyroglobulin itself. And the pitfalls I'm going to focus on are technical interferences and most importantly it's up to you, the physician, to suspect an interference because the hallmark of interference is discordance between the test result and the clinical presentation of the patient and this is information that the laboratory does not have. So it's critical for physicians to work with the laboratory to investigate any possible interferences. Now interferences can affect any endocrine test. Endocrine tests are typically made by immunoassay methodology that is prone to interferences. You should suspect an interference from discrepancies with past results for the patient, other laboratory tests or the clinical setting and only the laboratory can confirm an interference and this is often rarely identified by a single test. And importantly about 50% of cases of interference have led to inappropriate diagnosis or clinical management. So this is a very important issue. Now interferences can fall into two categories, either nonspecific interferences not related to the test analyte or specific interferences related to what you're testing for. Nonspecific interferences include heterophile or animal antibodies. I'm sure you've all heard of Hema, human anti-mouse antibody. Rheumatoid factor is considered heterophile antibodies. You can have antibodies targeting the test reagents and I'm sure you've all heard of high-dose dietary biotin that can interfere with some tests, but I want to mention that the manufacturers of the tests have been working very hard to eliminate this problem. And I think by about this time next year, we're going to have few if any tests that are going to be biotin sensitive. You can have mutant proteins or paraproteins that interfere. And then in terms of specific interferences, these are autoantibodies that bind the test analyte or molecular variants of the test analyte. So it's the inherent immunometric assay design that's basically used for all endocrine tests that makes these tests vulnerable to interferences. And what you have are two monoclonal antibodies. One has a signal, usually chemiluminescence, attached to it. The other is bound to some sort of solid support. And they've got different specificities to detect different epitopes on the test antigen. So that during incubation with a specimen, the antigen in this serum binds both monoclonal antibodies, forming a bridge that links the capture monoclonal to the signal monoclonal. And you can see that the specific epitopes are going to be different on each of these two monoclonals. So that after washing away the unbound constituents, the signal bound to the monoclonal antibody complex will be proportional to the serum antigen concentration. So let's first look at heterophile antibodies. And here on the left, you see the schematic that I've just shown you with the two monoclonals and the test analyte forming a sandwich in between when there's no interference. If you've got heterophile antibodies, quite often these can bind up both of the monoclonals and create a false signal even in the absence of analyte. So you might have a Graves hyperthyroid patient with a TSH of three and clearly inappropriate. And this would be the kind of result that would make you suspect heterophile antibodies. Rarely you can have so much heterophile antibody present. It binds up all the monoclonals and the analyte cannot form the reaction and you get a falsely low result. Now heterophile antibody interferences are both test and platform dependent. And this is illustrated by this case of a 33-year-old woman, no symptoms suggesting an endocrine problem, but has sera-contained heterophile antibodies that caused many endocrine test abnormalities. And here you see a range of different tests on the abscissa with the reference ranges in green. Her specimen was measured in the four major platforms we use in the US and you can see a lot of falsely high values. Now you might look at this data and say, well, the Roche test, the results that are shown in pink, really looks the best. But patient's heterophile antibodies are all different and the Roche might look good with this patient, but could look the worst with another patient. So now looking at antibodies that target test reagents and biotin can also be used as a reagent in these tests. And that is why when manufacturers use the strong biotin streptavidin relationship, there can be biotin interference from biotin the patient's taking or some patients can actually make streptavidin antibodies that interfere. Other tests use horseradish proxidase or ruthenium as reagents and patients can make antibodies against these reagents too that could cause interference. Looking at biotin, here you see the situation where biotin is part of the separation system and if the patient is not taking biotin supplementation, there's no problem. But if there is high dose dietary biotin present, then this can interfere with the separation system and a falsely low or high result can occur, which direction depends on the analyte and the type of method used. You can have mutant proteins or paraproteins. So here you see the total and free thyroid hormones and TSH and I want to make the point that when you check off free T4, free T3, these are not direct measurements of the thyroid hormones. They're estimate tests and they can be binding protein sensitive. So when you have paraprotein interferences, it could affect any of these tests but the test that's going to be reliable in this situation is free T4 by equilibrium dialysis, which is a test that's expensive. You would have to send it out usually to a reference laboratory, but this can usually get you some good results when you have this dilemma. Now, as you know, the thyroid hormones are bound to plasma proteins, primarily TBG, and if you've got abnormal binding proteins, maybe congenital TBG excess or deficiency or albumin variants, familial disalbuminemia can be a problem, then you're going to have high levels for both total thyroid hormones and the free hormone estimate tests. But of course, TSH is not going to be affected and your go-to test again is the free T4 by equilibrium dialysis. Now, familial disalbuminemic hypothyroxinemia is not really rare. It can be up to a prevalence of 1.8% in Hispanics and here you see four cases of the typical mutant that you might encounter. Again, direct equilibrium dialysis is going to give you a reliable result, but all these different manufacturers free T4 estimate tests tend to give you falsely high values. Turning now to interferences that are specific to the test analyte, molecular variance of the test analyte can be a problem and this is a problem again because of the inherent immunometric assay design. Remember, I mentioned the monoclonal antibodies target specific epitopes on the analyte and of course the epitopes are dependent on that analyte having the appropriate configuration so that the epitopes aren't masked or there are no strange epitopes that might bind. So if the test analyte is heterogeneous in the serum, there may be multiple analyte isoforms expressing different epitopes, some of which may not be recognized by the test monoclonal antibodies and this can lead to between method variability related to the different monoclonal antibodies used by the different manufacturers assays. So the tests that have molecular variance are primarily TSH, tharoglobulin, and tharoglobulin antibody and again these tests that have got heterogeneous isoforms with unusual molecular structures or conformation, they may not be detected at all by certain manufacturer's tests or may be detected differently and this is why you get different results in different tests from different manufacturers. And of course TSH is a case in point because it's a heterogeneous glycoprotein hormone. It circulates as a mixture of different glyco isoforms. Not all the TSH glyco isoforms are bioactive and it's hypothalamic TRH that influences TSH molecular glycosylation, molecular conformation, its metabolic clearance, and its bioactivity. And this molecular heterogeneity can cause discordance between TSH, you know, and bioactivity. For example in central hypothyroidism, TSH isoforms with impaired biologic activity are secreted and these are detected by normal, as normal TSH, by most of the immunometric assays and conversely when you've got TSH produced by a TSH secreting pituitary tumor, these molecules often have enhanced biologic activity. Again, they're usually detected by the TSH IMAs as regular TSH. So here you see a patient whose hyperthyroid has high levels of thyroid hormone, but the TSH can be paradoxically normal or even high instead of, as would be expected, low. So even when you've got an intact hypothalamic pituitary axis, this TSH heterogeneity can cause between-method variability. So this was a study of a number of different manufacturers' TSH tests and some manufacturers' methods run consistently lower than others all across the range, like the method shown here in blue. Other methods run consistently higher, like the method shown in red. And if you just look across the normal reference range, you can see there can be up to one milliunit per difference when measuring the same serum in different manufacturers' methods. And this is a problem establishing universal reference ranges for conditions like pregnancy. So this is a fairly recent study, which was a summary of 43 studies of each trimester of pregnancy, where the upper levels of TSH shown in pink and the lower levels in blue were plotted against the method. And you see the four major platforms that the clinical chemistry lab uses to measure TSH. Now all the methods show that, as expected, TSH tends to be lower in the first trimester and return to normal as pregnancy progresses. And I know the American Thyroid Association guidelines suggest that you can just perhaps use an upper limit of four for pregnancy. But really the variability you see here doesn't really suggest that this is a viable option. In pregnancy, 3T4 measurements are also very variable. This is the same study. And as you can see, the 3T4 tends to trend down as pregnancy progresses, as you would expect. But the variability is huge, both within the trimester and across the methods. So we're a long way from establishing universal reference ranges, even though the societies are pushing manufacturers to do this, but this is very slow to happen. Then turning to thyroglobulin, thyroglobulin antibody, these really have to be considered together because they're both tumor markers for differentiated thyroid cancer. In thyroglobulin antibodies, the sensitivity and specificity differs, and this is because there's patient-specific antibody heterogeneity. So in this study of four different thyroglobulin antibody methods measuring 144 patient sera, you can see that different methods report different numeric values that can be different by a hundredfold. The manufacturer recommended cutoffs for a positive thyroglobulin antibody are set too high. They're set for thyroid autoimmunity, not the level of antibody that's going to interfere with a thyroglobulin measurement. And of course, we don't typically use thyroglobulin antibody to assess thyroid autoimmunity. What needs to be used is to define a positive thyroglobulin antibody is the assay's functional sensitivity, and this is typically lower than the manufacturer cutoff. And then you can see there's method insensitivity. Sera can be judged antibody positive by one method and antibody negative by another. Then of course thyroglobulin in serum is heterogeneous. This is certainly the case for tumor neoplastic thyroglobulin isoforms, and this can cause a twofold difference in thyroglobulin if it's measured by different methods. So this is a study using C on the abscissa, different thyroglobulin methods. Each was measured by a bunch of different labs and they all received four specimens, A, B, C, and D. And you can see that whether you're measuring thyroglobulin in the high range A, medium range B, or the low range C and D, there can be a twofold difference in the thyroglobulin reported by different methods. And this is a problem because we monitor the trend of thyroglobulin in our cancer patients. We're interested in thyroglobulin doubling times because we know it has prognostic value, such as shown here. Those with a short doubling time have a worse prognosis than those with, say, a longer doubling time of one to three years. And of course most thyroid cancer patients do very well and the thyroglobulin does not rise. So then, finally, autoantibodies that bind the test analyte. And these are not rare. You can see that the prevalence of T4 and T3 autoantibodies can be up to about 1.8% in patients with thyroid autoimmunity, exactly the patients you're going to be measuring the thyroid hormone levels in. So you typically get a falsely high level. Again, free T4 by equilibrium dialysis can be very reliable. And of course the TSH will also be reliable in this situation. TSH autoantibodies, sometimes called macro TSH, are also not rare. The prevalence here can be up to about 1.6%. Of course, these are not going to affect your thyroid hormone measurements, but the TSH can be markedly elevated and really be very strange in this situation. And there's not a lot you can really do to identify whether there are macro TSH antibody interferences. The lab could precipitate it down with something like polyethylene glycol, but that's a lot of work. The lab doesn't get paid for it, and it is difficult to find somebody to do that. So turning back to thyroglobulin autoantibodies, these remain the major technical pitfall affecting the clinical utility of serum TG used to monitor cancer patients. We've got three different methodologies we use to measure thyroglobulin. Most thyroglobulins measured by the immunometric assay, they're rapid, automated, they use monoclonal antibodies with limited epitope detection, second-generation IMAs, which most are now, can measure down to 0.1, but they're prone to not only thyroglobulin antibody, but also heterophile antibody interferences. Radioimmunoassay, of course, is the older technique. There aren't many RIAs left, but they are resistant to thyroglobulin antibody interferences. They're not affected by heterophile antibodies, and they use polyclonal antibodies with broad epitope detection that better detect abnormal TG isoforms. And the most recent method are the liquid chromatography tandem mass spectrometry tests. These are free from heterophile antibody interferences, and they're being promoted as free from thyroglobulin antibody interferences, but as I'll show you, this is questionable. So this shows you the interferences aren't too problematic with the RIA, but the IMA, which is why most labs reflex to a method that they think is resistant to interferences. And this shows you the IMAs often have falsely undetectable values in over 50% antibody positive patients with disease. And the mass spec methods don't do much better. So what we tend to do is use the thyroglobulin antibody trend as a surrogate marker. So to close, suspect an interference when the test results unexpected, inconsistent with clinical or biochemical tests, odd results, patients with autoimmune or chronic diseases are more susceptible, and veterinarians also who are coming to contact with animals. So I thank you for your attention. Sorry, I'm a little bit over. No, thank you so much. We're gonna take a few minutes of questions for Dr. Spencer. If anyone wants to come up to the microphone and line up, I'll start. So for macro TSH and TSH auto antibodies, two parts. One, is it your experience that TSH is not just going to be a little elevated, but dramatically elevated? And do you find it can be moved at all by adjusting the thyroid medication in hypothyroid patients, or it's just immobile? It's not going to be elevated, and it isn't going to respond to thyroid hormone medications. Okay, no other questions? Yeah, go ahead. Thanks, Dr. Spencer. I had a patient come in who is u-thyroid clinically, and she was sent to me by her primary care doctor because her total T3 was 675, and her free T3 was 4.1 with the TSH of 2.0 and a free T4 of 1.2. Could you repeat the first two tests? Yes, her total T3 was 675. Yes. Free T3 was 4.2. Her free T4 was 1.2, TSH 2.0, and clinically u-thyroid, no prior history of thyroid disease, no history of autoimmunity, no family history. And no strange medications. No. Well, you would, I would guess the TSH is right based on the clinical presentation because there are so many things that can interfere. Were the antibodies, her TPO antibodies, were they detectable? I'm trying to remember. I think they were slightly elevated. I checked her anti-TG as well. I tend to do both. Anti-TG was not elevated. See, if there's evidence of thyroid autoimmunity, the risk for an autoantibody against the thyroid hormones also goes up. But it's such a high total T3. And is there, is there a condition called isolated hyper triiodothyronine I have not heard of that. I mean, it's mainly in Graves' disease that you get an increased T3-T4 ratio because of the TSH receptor antibodies stimulating. But it doesn't sound that that is the situation here. We'll do one more question for Dr. Spencer. You mentioned that the thyroglobulin antibodies are set too high for measurement of thyroglobulin, like situation with thyroid cancer. You said they should be lower. In the absence of this information, how low would you say this is negative? So let's say normal antibodies is 10 to 60, for example. Where would you, where would you say, look, below this is fine? It all depends on the method. Yeah. Because there can be, the absolute numeric value is just unique to that method. For example, if you're measuring the thyroglobulin antibody in the Roche method, anything above 20 is positive. Well, that value of 20 is less than one on the Beckman method. So it's very method dependent. You have to go by what the functional sensitivity of the method used by the laboratory is to know if it's very high or very low. Okay, thank you. So next we're going to be speaking about checkpoint inhibitors and thyroid dysfunction. Dr. Cotwell is a member of the endocrine oncology group and assistant professor at the University of Nebraska. His endocrine training was at the Mayo Clinic and his research interests include endocrinopathies from cancer therapies, thyroid nodules and thyroid cancers. After Dr. Cotwell, we will hear from Dr. Giuseppe Barbicino who is assistant professor of medicine at Harvard where he practices and teaches in the thyroid unit at Mass General. His research interests mainly involve understanding genetics of thyroid autoimmune disease and clinical aspects of thyroid cancer. His passion is in patient care and teaching the students, the fellows and the residents in the Harvard system. So Dr. Cotwell. Thank you for that introduction. So these are my disclosures and at the end of the talk, I hope you all will be able to diagnose and manage checkpoint inhibitor induced thyroid dysfunction, have an understanding about the pathogenesis of this and appreciate a connection between thyroid dysfunction and the efficacy of checkpoint inhibitors. So just very quickly, some basics about checkpoint and checkpoint inhibitors. Tumor cells, they present their antigens which are processed by antigen presenting cells which then present these to T cells that lead to its activation. Now activated T cells will express a checkpoint called CTLA-4 or cytotoxic T lymphocyte antigen four which then interferes with the binding between B7 and CD28. Basically what it does is represses the T cell activation early on in the immune system, predominantly in the lymph nodes. Now blocking this checkpoint with a monoclonal antibody will thus lead to enhanced T cell activation and proliferation. The activated T cells also show another checkpoint called programmed cell death protein one or PD1 which can bind to its ligands on peripheral tissue including the tumor cells and represses the T cell function in the periphery. So there are multiple monoclonal antibodies here with difficult to pronounce names that will block these checkpoints and lead to activation and enhanced T cell function. So it makes sense that this upregulation and activation of the immune system by these checkpoint inhibitor therapies will be very effective for tumor cell destruction and their indications for different cancers are increasing in number. But at the same time, they can cause a number of immune-related adverse events or IRAEs of which thyroid dysfunction is very common. So to show that, I will present case of a 39-year-old gentleman we took care of who had tremors, anxiousness, and palpitations a couple months after being on ipilimumab which is a CTLA-4 inhibitor and nivolumab, a PD1 inhibitor for his metastatic melanoma. He had tachycardia, a mild tremor, but no enlargement of thyroid gland or tenderness of the thyroid gland. So as you can see here, his thyroid function test before starting the checkpoint inhibitors were normal, but at the presentation, he had a suppressed TSH with increased thyroid hormone levels. His thyroid antibodies which were tested were negative. Ultrasound was done which showed a hypovascular and a heterogeneous thyroid gland which also had a reduced uptake on radioiodine scan in the absence of recent iodinated contrast. Now incidentally, he was noted to have diffused uptake on a PET scan which was done, the FTG PET scan being done for monitoring his melanoma. So with this whole presentation, this was felt to be a thyroiditis from the checkpoint inhibitors. He was managed conservatively and then over a few weeks, his TSH still remained suppressed, but T4 and T3 normalized, so he was monitored like that, but then developed new symptoms of headache, fatigue, nausea, and diarrhea, at which time, as you can see here, he had undetectable T4 and undetectable cortisol, ECTH, so was found to have hypopituitarism involving multiple pituitary hormones, and in comparison, showing a normal pituitary gland on your left as compared to an enlarged pituitary gland when he presented. So had both thyroiditis and hypophysitis from checkpoint inhibitors. So we'll touch upon the presentation, and to evaluate that, we investigated a PD-L1 inhibitor-treated cohort at Mayo Clinic, and we found that in this group, 25% of the patients developed a thyroid dysfunction, predominantly presenting with hypothyroidism. There were a few that presented with thyrotoxicosis, which either progressed to hypothyroidism or had normalization of the thyroid function. There were some patients that had worsening of their underlying hypothyroidism as well, requiring an increase in levothyroxine dose without any other etiology. So overall, about half of the patients that had presented with thyroid dysfunction ended up requiring thyroid hormone replacement due to overt hypothyroidism. So we validated these findings in a larger cohort, so I moved institutions after finishing my training. So at Nebraska, patients getting multiple immune checkpoint inhibitors. In that group, 17% overall developed a thyroid dysfunction, again, slightly lower number because we also had CTLA-4 inhibitors. And similar presentation, mostly presenting with hypothyroidism, the number that had thyrotoxicosis usually progressed to overt hypothyroidism. Some had euthyroidism, but there was one case that had persistent hyperthyroidism for about two and a half months and ended up requiring antithyroid medicine. So these adverse effects from the thyroid gland occur after a median of 2.4 months, and in this cohort, about two-thirds of patients that had new-onset thyroid dysfunction required thyroid hormone replacement. So these studies, as well as those done by others, show that thyroid dysfunction is the most common endocrine-adverse event from checkpoint inhibitors, usually occurs after a PD-1 or PD-L1 inhibitor or a combination of those with the CTLA-4. The median time to onset is six to 10 weeks for thyrotoxicosis, and eight to 16 weeks if it presents with hypothyroidism. Most patients are asymptomatic, so these are found usually by screening tests, or they may have mild symptoms initially if they have thyrotoxicosis or overt hypothyroidism. There can be different presentations which we went over, but important to note that there are some cases of persistent hyperthyroidism, even a couple cases with Graves' ophthalmopathy that have been reported, but that is usually rare. Overt hypothyroidism, if it develops, is usually permanent. So given the high frequency of these side effects, it makes sense that we should know, try figuring out ways to predict these. So there are multiple studies that have measured TPO or thyroperoxidase and TG or thyroglobulin antibody, usually at the time when the patient presents, so at the time of the thyroiditis, and these have been found to be present in a higher frequency than that seen in general population, but less frequent than typical Hashimoto thyroiditis. Now, some studies have evaluated these antibodies at baseline, so before starting checkpoint inhibitor, we summarized that in a table here as part of a commentary we did, but highlighted one of the studies by Japanese group showing a 34% risk of thyroid dysfunction if there were antibodies at baseline as compared to 2.4% in the absence of it. So some suggestion that antibodies at baseline do predict increased risk. Now, there have been other predictors also, so diffusely increased FTG uptake on a PET scan usually accompanies or precedes the thyroid hormone dysfunction. We found, and as well as a couple other studies, have found a higher TSH prior to the checkpoint inhibitor, even in the high normal range, is associated with a higher risk of thyroid dysfunction. One study looked at thyroid heterogeneity at baseline, so suggesting some worsening of underlying autoimmunity. Clinically, we see that patients who have severe thyrotoxicosis at presentation usually then become overtly hypothyroid. A few studies that have looked at other factors did not find a significant association with age, sex, or type of cancer when they evaluate for these factors in large cohorts. So I'll go over some algorithms for screening, diagnosis, and then management. These are based on our experience. We recently published a review on this, but then also taking into account a few of the guidelines in this area. So screening is fairly simple, measuring TSH and free T4, usually every four to six weeks for the first six months, and then every eight to 12 weeks thereafter, or if someone is noted to have incidentally increased FDG uptake in the thyroid or has symptoms of thyroid enlargement, which are rare. In addition to that, we also recommend checking a TPO antibody given the data I showed you, but again, this is a newer data out there, especially in someone who's starting a PD-1 or PD-L1 inhibitor on a combination because there's a much higher risk of thyroid dysfunction there. Now patients can present with thyrotoxicosis, primary hypothyroidism, or kind of a low normal TSH and low T4. A few points is that initially there can be fairly rapid release of thyroid hormones, so the TSH can be normal. In the setting of low TSH as well as low T4, this could be that the patient had thyrotoxicosis and we missed it, and they are now progressing to hypothyroidism, but again, it could be central hypothyroidism or non-thyroidal illness if someone is very sick. So the diagnostic workup includes monitoring of the thyroid function test, looking back to see what they were before presentation, checking a T3 if the TSH is suppressed, evaluating for other adverse events, drugs, biotin interference that can affect the thyroid function test, measuring the TPO and TG antibody, especially at the time of thyroid dysfunction, as we showed it is associated with the severity of thyroid dysfunction also. Ultrasound we reserve for cases that we are not sure about the diagnosis. We don't usually do it in all the cases. And then we do evaluation for Graves' disease only if there is persistent hyperthyroidism more than two months. So since most of these cases are thyroiditis, it would not be needed early on. The patient has central hypothyroidism, they need a pituitary workup. As for the management, thyrotoxicosis can usually be managed conservatively with a cardioselective beta blocker or NSAIDs if there is significant thyroid pain. It's important to monitor the TSH and T4 every two to four weeks because these patients can very quickly become hypothyroid. Persistent hyperthyroidism is rare, so if that's going on, they can be evaluated for Graves' disease. But usually the patients either become euthyroid or progress to overt hypothyroidism, in which case they should start on thyroid hormone replacement. Now if a patient has subclinical hypothyroidism, which is a high TSH but normal T4, the tests can be repeated to see for normalization. But if the patient has a persistently high TSH, more than 10, has symptoms, especially if that TPO antibody is positive, they're more likely to become overtly hypothyroid. So you can start with a low dose of levothyroxine to replace that. It's important for the clinician to keep in mind that the patients who are on thyroid hormone replacement, if not full dose, may require an increase in the dose and have worsening there. And then very important is to evaluate and start replacement with hydrocortisone if the patient has adrenal insufficiency, especially if they had central hypothyroidism. Monitoring thereafter is fairly typical, so every eight to 12 weeks until they are on a full replacement dose and then every six to 12 months. And then titrating the dose based on TSH for primary hypothyroidism and T4 for central. So a few key points about the management that is based on questions that come up. Usually the checkpoint inhibitor in this case does not need to be discontinued. It may need to be stopped temporarily for a severe side effect and then resumed. There has been conflicting practice as well as guideline recommendations about the dose of levothyroxine. So this is a study by the UCLA group recently that came out showing that patients who had thyrotoxicosis and then became overtly hypothyroid finally required a dose of 1.45 microgram per kg per day. So maybe in that group they can start with a slightly higher dose as shown here. But especially if we're evaluating elderly patients who have cardiac issues and if they're presenting with mild thyroid dysfunction, we usually start with a lower dose of 0.8 to 1.2 microgram per kg per day so as not to cause any side effects there. There is no role of glucocorticoids unless there is significant pain and enlargement, very rare cases. And similarly the antithyroid drugs are also required very rarely. So we'll move on to the pathogenesis and this is still being actively investigated but I will point a few key studies here. So the ligands for the checkpoint have been shown on thyroid cells. Cytology from a couple thyroid samples from patients with thyroiditis from checkpoint inhibitors showed infiltration with lymphohistiocytes, CD8 positive T-cells and the mouse models created for this condition have also shown infiltration with cytotoxic T-cells that express interferon gamma, granzyme B and interleukin 17. Now the MHC class II haplotype we know is associated with autoimmune thyroid disease, so Hashimoto's, and this is continuing to be investigated for thyroiditis from checkpoint inhibitors. So to look at some of these issues we at Mayo Clinic compared the immune infiltrate of patients with checkpoint inhibitor thyroiditis. We did fine needle aspirates of these patients and compared it to the healthy samples taken from the thyroid biopsy of patients who ended up having benign nodules. So I will not go into details here but basically what we found that patients with thyroiditis from checkpoint inhibitor had infiltration of T-cells, specifically those expressing CD8 positivity. Interesting subgroup was these double negative T-cells which have been associated with Hashimoto's thyroiditis and other autoimmune conditions, so some similarity there. But then also T-cells that express the checkpoint, so suggesting that there may be increased upregulation in the thyroid of these cells in patients that develop the thyroid dysfunction from the checkpoint inhibitors. Well we saw one of the HLA haplotype to be shared between six patients but again this is a small study. Since our study, the UCLA group by Dr. Lechner has again shown in addition to mice, in humans also this interleukin-17 inflammatory marker expressing T-cells, so giving further insight into the pathogenesis. But again clinically we can look at the different features of this thyroid dysfunction and know that the FDG uptake, the low uptake on a thyroid scan if done in appropriate setting, a thyroid ultrasound, they all point towards thyroiditis being the most likely cause. But it appears to have a faster progression from thyrotoxicosis to hypothyroidism than typical painless thyroiditis, so some difference there. We think the etiology is likely multifactorial, predominantly a destructive thyroiditis, but there are cases of worsening of underlying autoimmunity as well as rare cases of Graves' disease which may have a different mechanism that we don't know of yet. So we talked about presentation and the management of this very frequent thyroid dysfunction. Now there could be a silver lining here, and that is that are these thyroid side effects associated with efficacy of the checkpoint inhibitors? So to evaluate that we looked at cohorts both at Mayo and UNMC and we found that the patients that developed this thyroid dysfunction had improved overall survival as shown here. And then we adjusted for the age, sex, and the type of cancer and still found that patients that had thyroid dysfunction had almost 50% less risk of dying than patients that did not develop thyroid dysfunction while on the checkpoint inhibitors. So other studies have also looked at this and have actually found a link between the severity of the thyroid dysfunction and the survival, so suggesting that there's a link with that and the efficacy of the checkpoint inhibitors. Now there's this question of guarantee time bias because the thyroid dysfunction does occur after the patient is on checkpoint inhibitor, so some studies have looked at that also and still found an improved survival with this. So you don't need to read all of the data here, this is just showing the multiple studies that have been done in this area which we summarized in our review, showing anywhere from 40 to 70% lower mortality risk with checkpoint inhibitor-induced thyroid dysfunction. So at the beginning of the talk, I showed you all the divergence between the efficacy of checkpoint inhibitors and then the other side they cause multiple immune-related adverse events. But as we saw, the frequent occurrence of thyroid dysfunction from checkpoint inhibitors, the strong link with improved survival, and also now that these checkpoint inhibitors are being investigated for advanced endocrine cancer, this suggests that we should explore the convergence of these and really further investigate the immune interface of thyroid gland specifically in the area of thyroid cancer. So our team and others are working on that, that's a discussion for another talk. So with that, I'll just conclude with some take-home points. So there is a rising population burden of immune checkpoint inhibitor-induced thyroid dysfunction, so whether it be practicing endocrinologists or other clinicians, all of us will see these. And so this underlies the need for prompt recognition and management. Usually these occur early on after starting checkpoint inhibitor, but there are cases that can occur up to a year later as well, so we do have a heightened suspicion for these and we need to keep screening with thyroid function testing. They usually occur after the PD-1 or PD-L1 inhibitors, either alone or in combination. So the combination, in fact, causes a much higher risk, about 20 to 30% risk of the thyroid dysfunction, so something to keep in mind. The thyroid dysfunction most frequently presents as hypothyroidism or as mild thyrotoxicosis, so it is possible that a patient presenting with hypothyroidism may have had a very mild thyrotoxicosis that was just missed in between. The thyrotoxicosis usually rapidly progresses to hypothyroidism, so we need to monitor the levels quite frequently. And then usually if a patient is overtly hypothyroid, at least in most studies, they have not been able to come off thyroid hormone replacement there. This appears to be a T cell-mediated destructive thyroiditis. There appears to be a role of the thyroid peroxidase antibody both at the time of occurrence as well as at baseline in predicting the occurrence of this as well as the severity. But there are differences in mechanism as compared to autoimmune thyroid disease, so this is still being investigated. And then lastly, but very important, so the thyroid dysfunction appears to be a biomarker for the efficacy of checkpoint inhibitors. So this is still being explored, but I think the thyroid gland, especially affected by checkpoint inhibitors, serves as a good model to develop these novel immunotherapies, especially as we are considering them for advanced endocrine cancer. We're switching gears here in this interesting session, and I want to thank ACE for inviting me to speak here, and Dr. Margulis to chair this session. I'll be talking to you today about drugs and therodysfunction. Now, that's a very large topic, so rather than giving you in these minutes a long release of tables of what can happen, I thought I was gonna address only a few cases as examples, and leave it up for the future, hopefully if they invite me again, to complete that. I have really no disclosure related to this topic. Forget that last line, because I'm not gonna be talking about the use of this medicine for treating thyroid disorder. So let's start directly with a case. This is a 42-year-old female who has had a total thyroidectomy from thyroid cancer. She has been on levotaroxin, 200 micrograms for the past six years. She had an increase in those at some point, but not much. And she leaves a message to my phone saying, you know, I just checked my thyroid levels, and TSH is 45, and 3T4 is .6. She also said I have no brain fog, what? So I go to her record, and I look at her last visit. She was on the following medications, levotaroxin, 200 micrograms per day, calcium supplementation, PPI, HRT, and over-the-counter allergy medicine. So when you think about levotaroxin in efficacy with standard dosage in patients who are fully hypothyroid, meaning they have lost any residual type of function, we have to think about a number of possible. Non-adherence is certainly one of them. And although this is not the topic today, I really encourage you to think about this. All the times when unexplained elevation in TSH happen in patients who are taking levotaroxin, and the exercise I recommend you to do is to pick up the phone, call the pharmacy, and ask them how many refills the person has had in the past year, because you'll be very surprised, especially when there's no explanation. But let's leave it at that for now. Other possible options are malabsorption, and that can come either from primary disease in the GI tract, like celiac disease, short bowel, and a number of other conditions, but also by interferences by drugs in the absorption of levotaroxin. And here's one example. Calcium is known to affect the absorption of thyroxin. It does so by binding the levotaroxin in the GI tract. So they need to exist together in the stomach and intestine for that interference to happen. That binding results in the levotaroxin being excreted in the stools together with the calcium. Now if you look at this study in which they challenged the patient, hypothyroid patient, they gave him levotaroxin without calcium, in blue, and with calcium, in red, you can see that there is an effect, but the magnitude of this effect is not tremendous. If I have to guesstimate the area under the curve here, you would say that maybe about one third of the absorption of levotaroxin is affected by this. Note that unfortunately the y-axis starts at two and not at zero. My mistake, I apologize for that. But anyways, there is certainly a direct effect of calcium in the absorption of levotaroxin. And remember that our patient is actually taking calcium carbonate. The other thing that is interfering sometimes is the absence of gastric acidity. Levotaroxin requires acidity to be absorbed fully. And you see this is a seminal study that actually made it in the New England Journal of Medicine and it shows that when people are taking omeprazole, for example here, those of levotaroxin required to achieve complete suppression is higher than in patients who are not taking protonics, for example. Now this study doesn't allow you to estimate the magnitude of the problem because these were all patients who were euthyroid to start with. So if the magnitude of the problem is 100% malabsorption or 50% malabsorption, you won't see it in a study like this because patients would not become euthyroid. They would stay euthyroid. The goal here is suppressing TSH in patients with GOIT or something we used to do in the past we don't do anymore. But if you look at the effect in the general population on a variety of drugs that have commonly prescribed, this is a retrospective study, looking at large number of patients, the number here is very high, started on a variety of different medications while they're taking levotaroxin and they analyzed the effect of those on TSH levels six months later. And you can see that if you look at, for example, iron, PPIs, and calcium, they all cause a statistically significant increase in TSH in this patient.
