The goal of this PDSA (Plan-Do-Study-Act) cycle is to evaluate the effectiveness of different modifications made to chart notes for patients seen in the Gonda UCLA Diabetes Center at Santa Monica. The modifications include adding a hard stop or dot phrases to review flu and COVID vaccination completions in the history of present illness (HPI). In the previous cycle, it was observed that the use of dot phrases resulted in lower rates of review for both vaccinations compared to the hard stop. The completion rates for COVID and flu vaccinations varied between the two groups.<br /><br />Based on the data and reflection from the previous cycle, it was recommended to continue with a modified dot phrase template in the next cycle. This involved adding AACE/CDC patient education PDF forms into the after visit summary (AVS) to provide information to patients. In the current cycle, observations showed high rates of review for both vaccinations in the hard stop notes, while the dot phrase notes had lower rates of review. The completion rates for both vaccinations were consistent between the two groups.<br /><br />The planned change for the next cycle is to continue using hard-stop notes for providers who do not use the copy forward tool, educate providers on the importance of using dot phrases, and use dot phrases for patient education in AVS. The implementation of this plan will be carried out by clinicians in the diabetes clinic in February 2024. The change will be measured through chart reviews of documented review and completion of vaccinations, as well as AVS patient recommendations.<br /><br />The next cycle will involve continuing the use of hard stop notes and dot phrases, educating providers, and including AACE/CDC patient education PDF forms in AVS. If necessary, the notes will be adapted for new patient visits.
