The goal of this PDSA (Plan-Do-Study-Act) cycle is to evaluate the effectiveness of modifying clinic notes to include information about flu and COVID vaccinations. In the previous PDSA cycle, chart notes were modified to include a hard stop for reviewing vaccination completions, or alternatively, dot phrases were added to facilitate the review of vaccinations. The data from the fifth cycle showed that both the modified note and dot phrase groups had high rates of review for COVID and flu vaccinations, with no significant differences in vaccination completions between the two groups.<br /><br />In the sixth cycle, 90 patient chart notes were reviewed, and the data showed that both the modified note and dot phrase groups had high rates of review for COVID and flu vaccinations. However, vaccination completions varied, with the modified note group having higher rates of up-to-date COVID vaccinations but lower rates of up-to-date flu vaccinations compared to the dot phrase group.<br /><br />Based on the data and reflection from the previous cycle, the recommended adoptions for the sixth cycle include continuing to use a modified dot phrase template and adding AACE/CDC patient education PDF forms into the after visit summary (AVS). The plan is to continue using hard-stop notes for providers who do not use the copy forward tool in the EPIC system, provide further education on the use of dot phrases to providers who use the copy forward system, and use a dot phrase for patient education in the AVS.<br /><br />The next cycle will involve implementing these changes in the December 2023 clinic, both in-person and via telemedicine. The change will be measured through chart reviews of documented review and completion of vaccinations, as well as chart reviews of AVS patient recommendations.<br /><br />The goal is to determine if the modified clinic notes can be consistently used by other clinicians and if continued education and reminders will increase the percentage of clinic views. Furthermore, the effectiveness of the modified notes in increasing the review of risks and benefits of completing vaccinations will also be assessed.<br /><br />For the next cycle, hard stop notes will continue to be used for those not using the copy forward tool, and providers will be educated on the continued use of dot phrases for those using the copy forward tool. Additionally, hard stop notes will be used for new patient visits, and AACE/CDC patient education PDF forms will be included in the AVS.<br /><br />If necessary, the notes will be adapted for new patient visits in the future.
