The goal of PDSA cycle #1 was to improve vaccination documentation rates for patients with type 1 or type 2 diabetes in a diabetes clinic. The baseline data showed low rates of documentation for both flu and COVID vaccinations. The planned change was to use a modified clinic note that includes vaccination documentation and to standardize recommendations for vaccinations when they are not completed. The change was implemented in June 2023, and the implementation was measured through chart reviews of the lead clinician's notes. The implementation of a modified clinic note and standardized recommendations led to an increase in the rate of review and recommendations, but the goal of 100% was not met. Modifications will be made for the next cycle to continue improving the vaccination rates.
