The American Association of Clinical Endocrinology (AACE) is conducting Quality Improvement (QI) projects to advance adult immunizations. The health system partners involved in this project include The Cleveland Clinic Foundation, University of California Los Angeles, Harbor-UCLA Medical Center, Brown Medicine and Lifestyle Corporation, University of Nevada Las Vegas Health, and Prisma Health Center.<br /><br />The QI project deliverables are outlined on Powerpoint slides 9-16. In Month 1, the focus is on planning and analyzing vaccine policies and processes that impact patient care. The health system partner is required to assemble a project team, participate in monthly meetings with AACE, create accounts for access to AACE's communication platform, schedule internal project team meetings, collect QI education and project resources, and gather resources related to the patient experience with vaccines.<br /><br />Monthly reports and uploads are required to track progress. These reports should include a final roster of all staff involved in the project, a schedule for monthly internal project team meetings, vaccination policy and process documents used, materials and information patients receive related to vaccines, observations and analysis based on collected documents, and optional posting to the HSP Communication Platform.<br /><br />In Month 3, the focus is on identifying institutional efforts to increase vaccination rates for patients with diabetes and practices related to collecting data on social determinants of health. The health system partner is required to report on efforts aimed at increasing vaccination rates, describe the institutional efforts, analyze the efforts, and identify current practices related to social determinants of health.<br /><br />In Month 4, the focus is on assessing the ability to generate data that reflect metrics on implementing the CDC's Standards for Adult Immunization Practice (SAIP) and AACE's recommended vaccines. The health system partner needs to identify variables for inclusion in reports used for QI projects, identify potential QI projects, and determine the support needed for success.<br /><br />In Month 6, the health system partner is required to collect baseline data on practice related to AACE recommended vaccinations and CDC's SAIP using electronic health records and immunization information systems. Based on the data, they should plan initial quality improvement interventions.<br /><br />The application process for Round 3 of the Health System Partner program will open in November 2022, and the final applications are due by January 30, 2023. The selection criteria include geographic location, diverse patient population, and experience with electronic health records and immunization information systems.<br /><br />The next meeting is scheduled for December 7, 2022, at 5:00 pm CT, and further steps will be discussed at that time.
